In this lawsuit, we alleged that Grubhub charged consumers hidden fees and used deceptive marketing techniques to increase profits in violation of District of Columbia consumer protection laws. 

In order to provide you with your refund, you will need to fill out and return the enclosed W-9 form containing your social security number. We request that you return the W-9 within 60 days. We will keep your social security number confidential. Please submit your W-9 online, or return the completed W-9 form in the enclosed self-addressed envelope you received to:

Your refund, less any amount owed to the District of Columbia, should be available within four months after you sign and return the enclosed W-9 or complete the online claims process.
